Fellowship Program Details
The goal of the Pediatric Critical Care Medicine Fellowship Program is to provide balanced training in clinical pediatric critical care, and basic and clinical research. It is a 3-year training program, with 18 months designated to clinical rotations, and 18 months designated to research or other scholarly activity. Fellows interested in pursuing a physician-scientist career can apply for a 4th year sponsored by a NIH T32 training grant (see Research and Scholarly Activity). Time spent in clinical training is equally distributed over both ICU’s, guaranteeing exposure to a broad spectrum of patients. In addition, trainees spend one month with the Anesthesiology department at Children’s Hospital Oakland (typically early in the first year) and one month in the Adult ICU at UCSF. In addition, elective time is offered in the 3rd year of fellowship, during which trainees can spend additional time gaining knowledge and skills in areas of specific interest. The Children’s Hospital at UCSF has 23 ICU beds divided over In the 23-bed PICU at Children's Hospital of Oakland, the trainee is involved with the management of general medical and surgical patients, as well as a wide range of subspecialty patients, and is an integral member of the trauma team (for the only designated pediatric trauma center in the San Francisco/Oakland Bay Area). Subspecialty services frequenting the PICU include neurosurgery, cardiothoracic surgery, orthopedics, general surgery, cardiology, pulmonary, hematology, oncology, and bone marrow transplant. Both institutions have active transport programs, which reach north to the Oregon border, south to the Central Valley, and east to Nevada. Altogether, the trainee is exposed to the complete spectrum of pediatric diseases requiring intensive care management, and, through interaction with a wide diversity of subspecialists at both institutions, the trainee learns multiple different approaches to diagnosis and management. Research and other scholarly activities Approximately 18 months of fellowship training is available for research or other scholarly acitivities, for which the possibilities are extensive. Each Fellow gets an advisor assigned to help with the process of finding a research project and mentor. All pediatric fellows at UCSF participate in Fellows College, a professional development program, which provides additional guidance and mentorship as well as a series of seminars offered throughout the year. The Pediatric Critical Care Department at UCSF is affiliated with the Cardiovascular Research Institute of the University of California, San Francisco, which focuses its research on cardiovascular and pulmonary biology. Basic research, as well as clinical research, is within the Institutes scope. The Childrens Hospital Oakland Research Institute (CHORI) is a NIH-funded research institute affiliated with Childrens Hospital Oakland where the emphasis is on clinical and translational research in a variety of fields. These include cancer, obesity, sickle cell anemia, cystic fibrosis, HIV, and other infectious diseases. All Fellows at Childrens Hospital are invited to attend a monthly research-focused seminar covering topics such as biostatistics, literature review and study design. Although a wide variety of research opportunities exist within the above-mentioned institutes, there are also opportunities for clinical, translational and basic research at other research laboratories and institutes affiliated with UCSF and the University of California at Berkeley, as well as opportunities for trainees who are interested in pursuing a career as Academic Clinical Educator. Trainees interested in pursuing a physician-scientist career can apply for a 4-year program year sponsored by a NIH T32 training grant available within the division. This 4-year program includes 2 year protected research time; opportunities for both basic and clinical-translational research are available. Apart from the seminars mentioned above, educational activities during fellowship include a weekly conference, monthly Morbidity and Mortality conference, monthly journal club and monthly bedside teaching sessions. A comprehensive Training in Clinical Research course is offered every summer by the department of epidemiology and biostatistics at UCSF, with advanced training available for those interested in pursuing an academic career in clinical research. In addition, a myriad of lectures and courses are offered through Childrens Hospital Oakland Research Institute (CHORI), the Cardiovascular Research Institute (CVRI) and Fellows College, among others. See the down-loadable Fellowship Brochure (48MBs) for more information. The first year of the fellowship is focused on mastering clinical skills and knowledge, and on identifying a career focus. The trainee will spend one month in Pediatric Anesthesia and 7-8 months in the Pediatric Intensive Care Unit (equally divided over UCSF and Childrens Hospital Oakland). The remaining time is devoted to research/scholarly activities. During the second and third year of fellowship, progressively more time is set aside for research/scholarly activities, and less time is spent doing clinical work (5-6 months in the 2nd year, 1-2 months in the 3rd year). In addition to clinical time in the 2 PICUs, trainees spend one month in the Adult ICU at UCSF (2nd year), and one month in electives (3rd year). Each trainee has an advisor at either UCSF or CHRCO, with whom the trainee discusses progress, plans and problems in quarterly meetings. The advisor also helps the trainee in the process of identifying career goals, and finding a research project and mentor.
